Summary

A rare periclitoral pilonidal cyst requires thorough surgical resection for a permanent cure. This case demonstrates successful wide local excision, preserving the clitoris for complete healing.

Background:

Periclitoral pilonidal cysts are rare conditions characterized by sinus tracts containing hair follicles.
Diagnostic criteria involve inflammatory reactions within epithelialized sinus tracts.
Limited long-term follow-up data exist regarding optimal surgical management for permanent cure.

Observation:

A 30-year-old patient presented with a chronic, draining periclitoral abscess treated for two years with antibiotics and local incisions.
The patient underwent wide local excision of the cyst and associated sinuses extending into the periclitoral area and labia minora.

Findings:

Complete resection of the periclitoral pilonidal cyst and associated sinuses was achieved.
The patient experienced primary closure and successful healing post-surgery.
The clitoris, including the shaft and glans, was preserved during the procedure.

Implications:

Thorough surgical resection is essential for the diagnosis and curative therapy of periclitoral pilonidal cysts.
Surgical management can be accomplished while preserving the anatomical structures of the clitoris.
This approach offers a potential pathway to permanent cure for this rare condition.
