Summary

Primary omental pregnancy, a rare ectopic pregnancy, can cause severe hemoperitoneum. This case highlights its consideration in acute abdominal pain with intact adnexa.

Area of Science:

Background:

  • Ectopic pregnancies typically occur in the fallopian tubes.
  • Omental pregnancy is an exceptionally rare variant, often secondary to tubal implantation.
  • Primary omental pregnancy, without a preceding tubal event, is exceedingly uncommon.

Observation:

  • A 25-year-old woman presented with acute abdominal pain, distention, and anemia, mimicking a ruptured ectopic pregnancy.
  • Laparotomy revealed hemoperitoneum, but bilateral tubes and ovaries were anatomically intact.
  • The source of bleeding was identified as an omental pregnancy at 12 weeks gestation.

Findings:

  • The diagnosis of primary omental pregnancy was confirmed based on Studdiford's criteria.
  • Histological examination revealed neovascularization within the omental tissue, supporting the diagnosis.
