  • Imaging revealed a 60-mm brachiocephalic artery aneurysm with mural thrombus.
  • The aneurysm and right subclavian artery were accessed via median sternotomy.

Findings:

  • The surgical team employed cardiopulmonary bypass with pulsatile perfusion and mild hypothermia.
  • A Y-prosthetic graft was used for ascending aorta to right common carotid artery bypass.
  • The brachiocephalic artery and right subclavian artery were repaired with a 30-minute circulatory arrest period.

Implications:

  • This case demonstrates a feasible and effective surgical approach for brachiocephalic artery aneurysms.
  • The technique minimizes cerebral ischemic time, potentially improving patient outcomes.
  • Successful repair led to patient discharge within seven days post-operation.
