Varicocelectomy improves pregnancy rates in infertile men with clinical varicocele, regardless of paternal age. Advanced paternal age does not negatively impact reproductive outcomes following this surgical intervention for varicocele.
Area of Science:
- Reproductive Medicine
- Andrology
- Urology
Background:
- Clinical varicocele is a common cause of male infertility.
- Advanced paternal age is increasingly recognized as a factor influencing reproductive outcomes.
Purpose of the Study:
- To evaluate the impact of advanced paternal age on reproductive outcomes in men with clinical varicocele undergoing varicocelectomy.
- To assess the efficacy of varicocelectomy in infertile men aged 40 years and older.
Main Methods:
- Retrospective review of clinical records for 581 infertile men with clinical varicocele.
- Comparison of reproductive outcomes between men aged 40 years and older and those younger than 40 years.
- Analysis of spontaneous pregnancy rates post-varicocelectomy and comparison with age-matched controls.
Main Results:
- Men aged 40 years and older had a higher proportion of secondary infertility (43% vs. 19%).
- No significant difference in baseline sperm parameters or spontaneous pregnancy rates after varicocelectomy between older and younger men.
- Varicocelectomy significantly increased spontaneous pregnancy rates in older men compared to non-surgical controls (49% vs. 21%).
Conclusions:
- Paternal age does not adversely affect pregnancy outcomes after varicocelectomy for clinical varicocele.
- Varicocelectomy is a viable treatment option for infertility in older men with clinical varicocele.
- The study supports varicocelectomy for treating clinical varicocele and infertility in men of advanced paternal age.
