Background:

  • Conventional ultrasound (US) is a standard imaging modality for breast lesion assessment.

Purpose of the Study:

  • To compare the diagnostic performance of conventional US and US elastography for differentiating nonpalpable breast masses.
  • To assess if elastography can reduce benign biopsies using histological analysis as the gold standard.

Main Methods:

  • 100 women with nonpalpable breast masses underwent conventional US and elastography.
  • Two experienced radiologists analyzed images and classified lesions by malignancy suspicion.
  • Receiver operating characteristic (ROC) curve analysis was used to evaluate diagnostic performance.

Main Results:

  • Areas under the ROC curves (Az) were 0.901 for conventional US and 0.916 for elastography (p=0.808), indicating comparable performance.
  • For BI-RADS category 4a lesions, 44% (22/50) had an elasticity score of 1 and were all benign.

Conclusions:

  • US elastography demonstrates diagnostic performance similar to conventional US in differentiating nonpalpable breast masses.
  • BI-RADS category 4a lesions with an elasticity score of 1 may not require biopsy, potentially reducing unnecessary procedures.
