Summary

Water diffusivity in normal-appearing brain tissue is elevated in multiple sclerosis (MS) patients compared to healthy individuals, detectable by diffusion-weighted imaging (DWI). This indicates subclinical disease burden in MS patients.

Background:

  • Multiple Sclerosis (MS) is a chronic neurological disease.
  • Subtle pathological changes in normal-appearing brain tissue (NABT) contribute to MS disability.
  • Diffusion-weighted imaging (DWI) is a sensitive MRI technique.

Purpose of the Study:

  • To investigate differences in water diffusivity in NABT between MS patients and healthy controls.
  • To determine if DWI can detect subclinical pathology in MS.

Main Methods:

  • Conventional MRI and DWI were performed on 37 MS patients and 31 age- and sex-matched controls.
  • Quantitative diffusivity values were measured in normal-appearing white matter (NAWM) and gray matter (NAGM) using a standardized region of interest template.

Main Results:

  • MS patients exhibited significantly higher water diffusivity in NAWM and NAGM compared to controls (p < 0.0001).
  • Elevated diffusivity was observed in deep gray matter and some cortical gray matter regions in MS patients.

Conclusions:

  • Water diffusivity is increased in NABT of MS patients, suggesting subclinical neuropathology.
  • DWI is capable of quantifying MRI-undetectable pathology in NABT, reflecting disease burden in MS.
