Summary

Partial tooth loss during clenching can overload the temporomandibular joint (TMJ). Maintaining dental contact distribution prevents TMJ overload, while severe partial edentulism causes overload depending on contact type.

Area of Science:

  • Biomechanics of the masticatory system.
  • Finite element analysis in dental research.

Background:

  • The temporomandibular joint (TMJ) is susceptible to loading during masticatory functions.
  • Understanding the impact of partial edentulism on TMJ loading is crucial for dental health.

Purpose of the Study:

  • To investigate the hypothesis of loading-dependence on the temporomandibular joint (TMJ).
  • To analyze TMJ loading under experimentally induced partial edentate conditions during clenching.

Main Methods:

  • A detailed finite element model (FEM) of the TMJ was utilized.
  • Electromyography (EMG) data from 10 volunteers were collected with and without intraoral appliances simulating partial edentulism (PE).
  • EMG data were integrated into the FEM to analyze joint loading across various PE conditions.

Main Results:

  • Muscular activity decreased with the loss of dental contact.
  • Absence of posterior tooth contact resulted in temporomandibular joint (TMJ) overload.
  • Unilateral molar presence without posterior contact led to asymmetric overloading of the TMJ disc.

Conclusions:

  • Uniform dental contact distribution during clenching prevents TMJ overloading.
  • Severe partial edentulism can induce TMJ overload, with severity influenced by the remaining dental contact patterns.
