Bone Marrow Sampling and Transplants01:22

Bone Marrow Sampling and Transplants

Bone marrow transplant is a potential cure for several diseases, including cancer and specific genetic disorders. Notably, this procedure is applicable for patients suffering from aplastic anemia, certain types of leukemia, severe combined immunodeficiency disease (SCID), Hodgkin's disease, non-Hodgkin's lymphoma, multiple myeloma, thalassemia, sickle-cell disease, and certain cancers.
The transplant begins with high doses of chemotherapy and radiation treatment, which aim to destroy the...

Summary

Umbilical cord blood (CB) offers a valuable alternative to bone marrow transplants, improving donor availability and HLA compatibility. Double CB transplantation is a promising approach to overcome cell dose limitations for adult recipients.

Area of Science:

  • Hematology
  • Stem Cell Biology
  • Transplantation Medicine

Context:

  • Umbilical cord blood (CB) contains hematopoietic stem cells (HSCs) comparable to bone marrow stem cells.
  • The first successful familial CB transplant occurred in 1988, treating Fanconi's anemia.
  • CB banks have increased donor availability for bone marrow transplantation to 75% of patients.

Purpose:

  • To explore the potential of umbilical cord blood as a source of hematopoietic stem cells for transplantation.
  • To address the challenge of insufficient cell numbers in single CB units for adult transplantation.
  • To evaluate novel strategies for enhancing CB transplantation efficacy.

Summary:

  • CB transplantation offers advantages like easier access and greater HLA incompatibility tolerance, benefiting ethnic minorities.
  • A minimum of 3.7 x 10^7 mononucleated cells/kg is recommended for successful engraftment, a threshold often unmet by single CB units.
  • Investigated strategies include co-infusion of haploidentical or mesenchymal cells, a second CB unit, or ex-vivo expanded CB.

Impact:

  • Double CB transplantation is emerging as a highly promising strategy to address cell dose limitations.
  • Ongoing research aims to determine if double CB transplantation can improve the duration of aplasia post-transplant.
  • This research has the potential to expand the application of CB transplantation to adult patients.
