Abstract:
This article outlines the new National framework for NHS funded continuing care and NHS funded nursing care (Department of Health, 2007) and the implications for community nurses. Successful implementation is dependent upon good multidisciplinary assessment and person-centred care planning. Good practice in these areas is described. The role of the registered nurse in carrying out an assessment for NHS funded continuing care, the importance of record keeping and professional accountability are discussed.
