Summary

Subretinal fluid endothelin-1 (ET-1) levels negatively correlate with visual acuity (VA) in patients with rhegmatogenous retinal detachment (RD). Higher ET-1 levels, especially with proliferative vitreoretinopathy (PVR), indicate poorer visual outcomes post-surgery.

Background:

  • Endothelin-1 (ET-1) is a peptide implicated in various ocular pathologies.
  • Its role in rhegmatogenous retinal detachment (RD) and its impact on visual acuity (VA) require further investigation.

Purpose of the Study:

  • To determine the correlation between subretinal fluid (SRF) endothelin-1 (ET-1) levels and visual acuity (VA) in patients with RD.
  • To assess the influence of proliferative vitreoretinopathy (PVR) on this relationship.

Main Methods:

  • Prospective study of 66 patients (42-70 years) with unilateral RD undergoing scleral buckling surgery.
  • Radioimmunoassay used to measure immunoreactive ET-1 (IR-ET-1) in plasma and SRF.
  • Visual acuity (VA) measured preoperatively and 8 months postoperatively.
  • Pearson correlations analyzed between IR-ET-1 levels and VA outcomes.

Main Results:

  • Significant negative linear correlations found between plasma and SRF IR-ET-1 levels and preoperative VA, postoperative VA, and VA difference.
  • SRF IR-ET-1 showed stronger negative correlations with postoperative VA and VA difference when RD was associated with PVR.
  • Mean SRF IR-ET-1 levels (10.71 pg/ml) were higher than plasma levels (2.91 pg/ml).

Conclusions:

  • SRF ET-1 levels are significantly associated with poorer visual outcomes in RD patients.
  • The correlation is stronger in cases with coexisting PVR.
  • Findings suggest potential therapeutic targets and support primary vitrectomy for RD management.
