This study presents an accurate frontal targeting method for hypertensive putaminal hemorrhage (HPH) using 3D CT scans. The minimally invasive neuroendoscopic technique achieved 84% hematoma evacuation with rapid patient recovery.
Area of Science:
- Neurosurgery
- Radiology
- Medical Imaging
Background:
- Hypertensive putaminal hemorrhage (HPH) is a severe neurological condition.
- Effective surgical targeting is crucial for successful treatment of HPH.
- Minimally invasive techniques are sought to improve patient outcomes.
Purpose of the Study:
- To develop and evaluate an accurate frontal targeting method for neuroendoscopic treatment of HPH.
- To assess the safety and efficacy of the described targeting technique.
- To determine the feasibility of using 3D reconstructed CT for surgical planning.
Main Methods:
- Development of a frontal targeting method using 3D reconstructed CT scans.
- Neuroendoscopic treatment of five HPH patients via frontal trephination.
- Independent neuroradiological analysis of CT images for trajectory planning.
- Measurement of entry point, depth, and trajectory based on orbitomental lines.
Main Results:
- All five patients regained consciousness post-operatively without complications.
- Accurate surgical trajectory was achieved in all cases.
- Average hematoma evacuation rate was 84%.
Conclusions:
- The 3D reconstructed CT-based frontal targeting method is accurate and effective for HPH.
- This technique offers a minimally invasive, time-saving approach for neuroendoscopic HPH treatment.
- The method eliminates the need for additional CT scans for surgical planning.
