    Summary

    Pregnancy does not halt ovarian follicular development. Instead, the hormonal environment of gestation appears to trigger premature degeneration of ovarian follicles and oocytes in women.

    Background:

    • Ovarian quiescence during pregnancy is widely accepted.
    • Superficial ovarian follicles are abundant in preparturient women.

    Purpose of the Study:

    • To investigate the status of ovarian follicles, oocytes, and cumuli in preparturient women.
    • To determine if pregnancy hormones prevent follicular atresia.

    Main Methods:

    • Examined 298 ovarian follicles from 30 women undergoing cesarean section.
    • Utilized Nomarski differential interference microscopy.
    • Assessed follicular size, oocyte status, and cumulus oophorus integrity.

    Main Results:

    • No oocyte was recovered from 50% of follicles.
    • 79% of recovered oocytes and 78% of cumuli showed signs of degeneration.
    • Degeneration correlated with the presence of phagocytes in follicular fluid.

    Conclusions:

    • The endocrine status of pregnancy does not inhibit early follicular development.
    • Gestation appears to induce premature follicular atresia, challenging the notion of ovarian quiescence.
