Acne Infection01:27

Acne Infection

Acne is a multifactorial skin condition primarily affecting adolescents and young adults, with a global prevalence estimated to exceed 75% in this demographic. The condition is characterized by the formation of comedones (blackheads and whiteheads), papules, pustules, nodules, and, in severe cases, cysts, particularly in areas rich in sebaceous glands such as the face, neck, chest, and back. The pathogenesis involves increased sebum production, follicular hyperkeratinization, colonization by...
Antiprotozoal Agents01:21

Antiprotozoal Agents

Leishmaniasis is a widespread parasitic disease caused by several Leishmania species. It affects millions of people each year and remains a major public health problem in endemic regions. First-line treatment relies on pentavalent antimonials, including meglumine antimoniate and sodium stibogluconate. Even so, how these drugs work has not been fully clear, especially their interaction with parasite-specific biochemical pathways. One key target is trypanothione reductase (TR), an enzyme that...
Phase II Reactions: Acetylation Reactions01:24

Phase II Reactions: Acetylation Reactions

Acetylation, a phase II biotransformation reaction, introduces an acetyl group to drugs or their metabolites. Acetyltransferase enzymes facilitate this reaction, which resembles α-amino acid conjugation due to the addition of a functional group to the drug molecule.
The substrates for acetylation are typically drugs or their metabolites with an amino, sulfonamide, or hydrazine functional group. Acetylation can occur at several points in the drug molecule, including primary, secondary, and...

Summary

Retinoid therapy, including etretinate and acitretin, effectively manages lichen amyloidosus symptoms like itching and papules. Continuous treatment is crucial, as discontinuing acitretin leads to symptom recurrence.

Area of Science:

  • Dermatology
  • Pharmacology

Background:

  • Lichen amyloidosus is a chronic skin condition characterized by itchy, hyperkeratotic papules.
  • Treatment options for recalcitrant cases remain limited.

Observation:

  • Two patients with extensive, long-standing lichen amyloidosus were treated with oral retinoids.
  • One patient received etretinate followed by long-term acitretin; the other received a course of acitretin.

Findings:

  • Long-term oral acitretin therapy controlled pruritus and flattened papules in a patient with generalized lichen amyloidosus.
  • Withdrawal of acitretin resulted in symptom flares, which resolved upon reintroduction.
  • A shorter course of acitretin showed marked improvement, but symptoms recurred after discontinuation.

Implications:

  • Oral retinoids, particularly acitretin, demonstrate significant efficacy in managing lichen amyloidosus.
  • Sustained retinoid treatment appears necessary for long-term disease control.
  • Further research into optimal dosing and duration for lichen amyloidosus is warranted.
