Background:

Foreign body aspiration is a rare but serious complication.
Subcutaneous emphysema can result from various causes, including airway obstruction.

Observation:

A 64-year-old woman developed massive subcutaneous emphysema.
The emphysema appeared two weeks after she unknowingly inhaled a Brazil nut.

Findings:

The case demonstrates a delayed presentation of foreign body inhalation.
Computed tomography (CT) scanning of the chest is crucial for diagnosing inhaled foreign bodies.

Implications:

Foreign body inhalation should be considered in the differential diagnosis of patients presenting with spontaneous subcutaneous emphysema.
Prompt diagnosis and removal of inhaled foreign bodies are essential to prevent complications.
