Summary

Patient beliefs about continuous positive airway pressure (CPAP) therapy for obstructive sleep apnoea (OSA) significantly predict adherence. Psychological factors, not just biomedical data, are key to improving CPAP use.

  • Biomedical indicators alone have limited success in predicting CPAP adherence.

Purpose of the Study:

  • To evaluate the predictive power of the health belief model on CPAP adherence.
  • To compare the contribution of psychological constructs versus biomedical indices in predicting CPAP adherence.

  • A cohort of 77 treatment-naïve OSA patients completed baseline questionnaires assessing health beliefs (outcome expectancy, self-efficacy, functional outcomes, risk perception).
  • Physiological data from diagnostic sleep studies were collected.
  • CPAP adherence was measured at 4-month follow-up.

Main Results:

  • Health belief model constructs independently explained 21.8% of the variance in CPAP adherence.
  • Combined psychological and biomedical factors explained 31.8% of adherence variance.
  • Higher outcome expectancies, greater sleepiness-related functional limitations, and lower perceived risk were associated with better adherence.

Conclusions:

  • Patients form expectations about OSA and CPAP treatment prior to therapy initiation.
  • These pre-treatment beliefs are significant predictors of subsequent CPAP adherence.
  • Targeting psychological constructs may improve adherence to CPAP therapy.
