General practitioners achieved cardiovascular preventive care targets in only three out of ten indicators. Significant variations in care quality were observed across the nine audited Malaysian general practice clinics.
Area of Science:
- Public Health
- General Practice
- Preventive Medicine
Background:
- Cardiovascular disease (CVD) poses a significant public health challenge, contributing to substantial morbidity and mortality.
- General practitioners (GPs) are pivotal in CVD prevention due to their community-based, continuous patient care.
- Effective preventive strategies within primary care are crucial for mitigating CVD burden.
Purpose of the Study:
- To evaluate the quality of cardiovascular disease preventive services provided in Malaysian general practice settings.
- To assess the adherence to established standards for CVD prevention through a medical audit.
Main Methods:
- A retrospective medical audit was conducted on patient records from nine Malaysian general practices.
- The audit included patients aged 45 and above who attended clinics in June 2005.
- Preventive care adequacy was measured against predefined criteria and standards.
Main Results:
- High achievement rates were noted for blood pressure recording (92.4%) and blood sugar screening (72.7%).
- Seventy-one percent of hypertensive patients achieved target blood pressure (<140/90 mmHg).
- Performance varied widely, with achieved standards ranging from 11.1% to 66.7% for other indicators like registry maintenance and lipid screening.
Conclusions:
- The audited general practices met targets for only 30% of the assessed cardiovascular preventive care indicators.
- Substantial disparities in the quality of preventive care were evident among the participating clinics.
- There is a need to improve the consistency and comprehensiveness of CVD preventive services in primary care.
