Malignant glaucoma, a rare complication of keratomycosis (fungal eye infection), can be treated with keratoplasty and antifungals. One case showed new malignant glaucoma after keratoplasty.

Background:

Malignant glaucoma is a severe complication of keratomycosis (fungal eye infections).
It presents with elevated intraocular pressure, shallow anterior chamber, and a fungal-iris exudate mass.

Observation:

Three cases of keratomycotic malignant glaucoma were analyzed.
All patients had a pupillary size of 4 mm or less and Fusarium infection.
One patient developed malignant glaucoma post-keratoplasty.

Findings:

Two cases achieved successful treatment via therapeutic keratoplasty, lens extraction, and systemic antifungals.
The occurrence of malignant glaucoma following therapeutic keratoplasty is a novel observation.
