Background:

  • Alopecia areata is a non-scarring hair loss condition affecting 0.15% of the population.
  • It can manifest as patchy hair loss (alopecia areata), total scalp hair loss (alopecia totalis), or complete body hair loss (alopecia universalis).
  • While often self-limiting, hair loss significantly impacts patients' social and emotional well-being.

Purpose of the Study:

  • To evaluate the efficacy of various interventions for managing alopecia areata, alopecia totalis, and alopecia universalis.
  • To synthesize findings from randomized controlled trials (RCTs) on alopecia treatments.

Main Methods:

  • Comprehensive literature search of multiple databases (Cochrane Skin Group, MEDLINE, EMBASE, etc.) up to February 2006.
  • Inclusion of randomized controlled trials assessing topical and systemic treatments for alopecia.
  • Data extraction and quality assessment by two independent reviewers, with contact to trial authors for additional information.

Main Results:

  • Seventeen trials involving 540 participants were analyzed, evaluating interventions like corticosteroids, ciclosporin, photodynamic therapy, and minoxidil.
  • No significant hair growth benefit was observed for any intervention compared to placebo.
  • No studies reported on patient-assessed hair growth or quality of life.

Conclusions:

  • Few alopecia areata treatments have been rigorously evaluated in RCTs.
  • Commonly used treatments such as corticosteroids and minoxidil lack convincing long-term efficacy data, despite appearing safe.
  • There is a critical need for large, well-conducted studies to assess long-term treatment effects on quality of life, considering spontaneous remission and alternative management options like wigs.
