Summary

Radiation therapy effectively relieved pain in most shoulder impingement cases, particularly secondary impingement. Primary impingement and acute shoulder injuries showed limited benefit from this treatment.

Background:

Degenerative shoulder diseases were previously grouped as 'periarthritis humeroscapularis'.
Current understanding differentiates primary (osseous narrowing) and secondary (swelling tissue) impingement syndromes.

Purpose of the Study:

To accurately diagnose shoulder diseases for targeted therapy.
To classify treatment outcomes using the Constant score.

Main Methods:

102 patients with 115 shoulder conditions received two radiation therapy (RT) series (6 x 0.5 Gy).
Total dosage was 6.0 Gy, with a 6-week interval between series.
Shoulders were assessed pre-RT, 6 weeks post-RT, and during a 2003 follow-up.

82% (94/115) of shoulder joints experienced pain relief at 18-month median follow-up.
Secondary impingement showed a significantly better response to RT than primary/non-impingement.
Conditions like tendinosis calcarea, bursitis, biceps tendovaginitis, and capsulitis adhaesiva responded well.

Conclusions:

Radiation therapy is effective for shoulder diseases associated with secondary impingement.
Limited or no benefit was observed for primary impingement syndrome, rotator cuff tears, or acute shoulder injuries.
