Summary

Radiation therapy effectively relieved pain in most shoulder impingement cases, particularly secondary impingement. Primary impingement and acute shoulder injuries showed limited benefit from this treatment.

  • Current understanding differentiates primary (osseous narrowing) and secondary (swelling tissue) impingement syndromes.

Purpose of the Study:

  • To accurately diagnose shoulder diseases for targeted therapy.
  • To classify treatment outcomes using the Constant score.

Main Methods:

  • 102 patients with 115 shoulder conditions received two radiation therapy (RT) series (6 x 0.5 Gy).
  • Total dosage was 6.0 Gy, with a 6-week interval between series.
  • Shoulders were assessed pre-RT, 6 weeks post-RT, and during a 2003 follow-up.

  • 82% (94/115) of shoulder joints experienced pain relief at 18-month median follow-up.
  • Secondary impingement showed a significantly better response to RT than primary/non-impingement.
  • Conditions like tendinosis calcarea, bursitis, biceps tendovaginitis, and capsulitis adhaesiva responded well.

Conclusions:

  • Radiation therapy is effective for shoulder diseases associated with secondary impingement.
  • Limited or no benefit was observed for primary impingement syndrome, rotator cuff tears, or acute shoulder injuries.
