Summary

Biliary fistulas, a complication of gallbladder removal, were successfully treated using modern endoscopic and percutaneous drainage techniques. These minimally invasive methods, combined with antibiotics and nutritional support, offer effective management for these complex cases.

Area of Science:

  • Gastroenterology
  • Digestive Surgery
  • Interventional Radiology

Background:

  • Biliary fistula is a recognized complication following cholecystectomy.
  • Traditionally managed surgically, new minimally invasive techniques have emerged.
  • Associated factors include retained stones, surgical trauma, and infection.

Purpose of the Study:

  • To report on the management of biliary fistula cases.
  • To evaluate the efficacy of combined endoscopic and percutaneous techniques.
  • To analyze patient demographics and fistula etiologies.

Main Methods:

  • Seven patients diagnosed with biliary fistula between 1984-1990 were analyzed.
  • Abdominal collections were managed with percutaneous drainage (Ring-McLean, Van Sonnenberg tubes).

Related Experiment Videos

  • Fistulas were occluded using biliary prostheses (stents) via endoscopic, percutaneous, or mixed approaches.
  • Main Results:

    • Etiologies included pancreatitis, cystic duct ligation issues, common duct injury, and choledochojejunostomy dehiscence.
    • Fistula types comprised choledochocutaneous, hepatocutaneous, and hepatoduodenal.
    • Percutaneous drainage successfully managed associated collections; stent patency ranged from 2 weeks to 24 months.

    Conclusions:

    • Modern endoscopic and percutaneous techniques, alongside antibiotics and nutritional support, provide effective treatment for biliary fistulas.
    • Minimally invasive approaches offer viable alternatives to traditional surgery for managing biliary complications.
    • These methods demonstrate successful fistula occlusion and management of associated collections.
