Insights
Aortic valve papillary fibroelastoma and coronary artery disease were diagnosed in a patient. Surgical excision of the mass and coronary artery bypass grafting were successfully performed.
Area of Science:
- Cardiovascular Medicine
- Cardiac Surgery
- Oncology
Background:
- A 75-year-old male with a history of myocardial infarction and transient ischemic attack underwent preoperative evaluation.
- The patient was referred for evaluation before a cystectomy.
Observation:
- Echocardiography revealed a mobile mass on the aortic valve's right coronary cusp.
- Multislice CT demonstrated significant narrowing of the proximal left anterior descending coronary artery.
Findings:
- Diagnosis of aortic valve papillary fibroelastoma concurrent with coronary artery disease.
- Surgical management included excision of the aortic valve mass and coronary artery bypass grafting using the left internal mammary artery.
Implications:
- Highlights the importance of comprehensive cardiac evaluation in patients with cardiovascular risk factors undergoing non-cardiac surgery.
- Demonstrates successful surgical management of a rare cardiac tumor alongside significant coronary artery disease.
- Emphasizes the potential for co-existing cardiac pathologies requiring multi-disciplinary surgical approaches.
Background:
A 75-year-old male with a history of myocardial infarction and recent transient ischemic attack was referred to a cardiology clinic for preoperative evaluation before a cystectomy. Transthoracic and transesophageal echocardiography revealed a mobile mass on the right coronary cusp of the aortic valve. Multislice CT demonstrated a significant narrowing in the proximal left anterior descending coronary artery.
Investigations:
Physical examination, chest radiography, laboratory testing, electrocardiography, transthoracic and transesophageal echocardiography, multislice-CT coronary angiography, pathological and histological examination of the surgically excised tissue.
Diagnosis:
Papillary fibroelastoma of the aortic valve in conjunction with coronary artery disease.
Management:
Surgical excision of the mobile lesion and an end-to-side anastomosis of the left internal mammary artery to the left anterior descending coronary artery.

Definition and Purpose
An X-ray, or radiograph, is a non-invasive method that uses ionizing radiation to take images of internal structures. It is mainly used in cardiac imaging to examine the heart, lungs, and major blood vessels, aiming to identify abnormalities in the heart's size, shape, and position, such as heart failure, congenital defects, and vascular...
