Summary

Segmental glomerular tip lesions, previously seen in adults, were identified in a small percentage of childhood minimal change nephropathy biopsies. This finding may not significantly alter clinical management, as outcomes often resemble typical minimal change nephropathy.

Background:

  • Segmental glomerular lesions at the tubular opening (tip changes) are recognized in adult kidney disorders.
  • Some adult cases initially diagnosed as minimal change nephropathy exhibit these tip changes.

Purpose of the Study:

  • To investigate the occurrence of glomerular tip changes in pediatric patients diagnosed with minimal change nephropathy.
  • To determine if these lesions impact the clinical course and management of childhood minimal change nephropathy.

Main Methods:

  • Retrospective review of 50 consecutive renal biopsies from 49 children diagnosed with minimal change nephropathy.
  • Histopathological examination to identify segmental glomerular lesions, specifically tip changes.

Main Results:

  • Segmental lesions were observed in 5 out of 50 pediatric biopsies (10%).
  • Four biopsies showed isolated tip changes, with most patients achieving remission.
  • One case with chronic hepatitis B showed persistent proteinuria and multifocal segmental lesions.

Conclusions:

  • Childhood minimal change nephropathy series may include cases of glomerular tip lesions.
  • The presence of tip changes in this context is unlikely to necessitate a change in clinical practice.
