Summary

Optimal diffusion-weighted imaging (DWI) for detecting transient global amnesia (TGA) lesions involves higher b-values and delayed imaging. Follow-up DWI several days after symptom onset significantly improves lesion detection in TGA patients.

Background:

  • Transient global amnesia (TGA) is characterized by temporary memory loss.
  • Diffusion-weighted imaging (DWI) can reveal punctate hyperintense lesions in the hippocampus associated with TGA.

Purpose of the Study:

  • To determine the optimal DWI protocol for detecting hippocampal lesions in TGA.
  • To investigate the impact of various imaging parameters and timing on lesion detection.

Main Methods:

  • Sixteen TGA patients underwent DWI within 24 hours and 3 days post-onset.
  • Four DWI sequences with varying b-values (1000, 2000, 3000) and section thicknesses (3, 5 mm) were used.
  • Lesion detection was compared visually across different sequences and time points.

Main Results:

  • 81% of patients exhibited hippocampal hyperintense lesions.
  • Higher b-values (2000/3 mm and 3000/3 mm) and delayed imaging (3 days post-onset) yielded the highest lesion detection rates.
  • Follow-up DWI significantly increased the number of detected lesions compared to initial scans.

Conclusions:

  • DWI protocols with b = 2000/3 mm or b = 3000/3 mm at 3 days post-onset are optimal for TGA lesion detection.
  • Follow-up DWI is recommended when initial scans within 24 hours are negative for lesions.
