Background:

  • The American College of Radiology Appropriateness Criteria (ACRAC) provide evidence-based guidelines for medical imaging.
  • Medicare payment policies may not fully align with these established criteria for neurologic imaging procedures.

Purpose of the Study:

  • To examine the relationship between the ACRAC appropriateness levels for neurologic imaging and Medicare payment determinations.
  • To determine if Medicare is more likely to pay for imaging procedures as their appropriateness level increases.

Main Methods:

  • A cross-sectional study analyzed 2,510 medical condition/imaging procedure combinations using ACRAC, ICD-9-CM, and CPT codes.
  • Data included Medicare Part B payment determinations for Florida and Connecticut.
  • Multivariate logistic regression was used to assess the impact of appropriateness on payment likelihood.

Main Results:

  • Payment likelihood increased with higher appropriateness levels: 61% for low, 70% for middle, and 74% for high.
  • Middle appropriateness increased payment odds by 48% compared to low (OR=1.48, 95% CI: 1.19-1.84).
  • High appropriateness significantly increased payment odds by 125% compared to low (OR=2.25, 95% CI: 1.66-3.04).

Conclusions:

  • Medicare payment determinations for neurologic imaging are associated with evidence-based appropriateness criteria.
  • Aligning Medicare payment policies with ACRAC can incentivize evidence-based practice.
  • Updating payment policies to reflect current appropriateness information is recommended for improved healthcare alignment.
