Summary

Antibiotic treatment for ventilator-associated tracheobronchitis (VAT) significantly increases ventilator-free days and reduces subsequent pneumonia and ICU mortality. However, it does not shorten the overall duration of mechanical ventilation.

Purpose of the Study:

  • To evaluate the impact of antibiotic treatment on mechanical ventilation duration in patients with VAT.
  • To assess secondary outcomes including subsequent ventilator-associated pneumonia (VAP) and intensive care unit (ICU) mortality.

Main Methods:

  • A prospective, randomized, controlled, multicenter study.
  • Patients with VAT were randomized to receive 8 days of intravenous antibiotics or no antibiotics.
  • Exclusion criteria included prior VAP or severe immunosuppression.

Main Results:

  • Antibiotic treatment was associated with significantly more ventilator-free days (12 vs. 2 days).
  • Subsequent VAP rates were significantly lower in the antibiotic group (13% vs. 47%).
  • ICU mortality was significantly reduced with antibiotic treatment (18% vs. 47%).

Conclusions:

  • Antimicrobial therapy for VAT improves ventilator-free days and reduces VAP and ICU mortality.
  • Total duration of mechanical ventilation was not significantly impacted by antibiotic treatment.
