Background:

  • Hyperacute rejection (HAR) is a known risk in kidney and heart transplants.
  • It is exceptionally rare in lung transplantation, with only four previous cases documented.

Observation:

  • A patient with end-stage COPD underwent a left lung transplant.
  • Within an hour of reperfusion, severe hypoxia and lung opacification occurred.
  • Despite interventions, the patient experienced multi-organ failure and died within 24 hours.

Findings:

  • Retrospective antibody screening revealed recipient sensitization to donor HLA antigens (HLA-I and HLA-II).
  • The patient had anti-A2 antibodies, and the donor expressed the A2 antigen, confirming ABO incompatibility.
  • Autopsy confirmed patent anastomoses and no signs of ischemia, ruling out other causes of graft failure.

Implications:

  • This case highlights the potential for ABO-incompatible lung transplants to cause hyperacute rejection.
  • Early detection of specific antibodies and HLA matching are critical for successful lung transplantation.
  • Increased lung transplant volumes may lead to a rise in HAR cases, necessitating improved diagnostic and management strategies.
