Purpose of the Study:

  • To evaluate the relationship between the utilization of diabetes flow sheets and patient care outcomes within primary care practices.
  • To determine if diabetes flow sheet use correlates with adherence to evidence-based guidelines for diabetes care.

Main Methods:

  • Medical records of 1,016 patients with diabetes from 54 family practices were analyzed.
  • Diabetes flow sheet usage was documented for each patient record.
  • Guideline adherence scores (0-100) for assessment, treatment, and target attainment were calculated; generalized linear models identified associations with flow sheet use.

Main Results:

  • Diabetes flow sheets were utilized in 23% of patient records.
  • Flow sheet use was significantly associated with higher adherence scores for diabetes assessment (55.38 vs. 50.13, P=.02) and treatment (79.59 vs. 74.71, P=.004).
  • No significant association was found between flow sheet use and the attainment of intermediate diabetes outcome targets (HbA1c, LDL, blood pressure).

Conclusions:

  • Diabetes flow sheets can effectively enhance adherence to clinical guidelines for diabetes assessment and treatment.
  • Further research is necessary to understand patient and physician factors influencing the link between flow sheet use and achieving diabetes outcome targets.
