Summary

A collagen shield rapidly healed a persistent shield ulcer in a child with vernal keratoconjunctivitis. This treatment resolved the rare corneal manifestation, leaving minimal opacity.

Background:

  • Vernal keratoconjunctivitis (VKC) is a chronic allergic eye disease.
  • Shield ulcers are a rare, potentially vision-threatening complication of VKC, often associated with mechanical irritation from large papillae.

Observation:

  • A 9-year-old boy with a 3-year history of VKC developed a persistent superior corneal shield ulcer.
  • Medical management with topical steroids and cromolyn sodium relieved itching and discharge but not the shield ulcer.

Findings:

  • A 72-hour collagen shield application was used to treat the persistent vernal shield ulcer.
  • The shield ulcer demonstrated rapid healing within 5 days of collagen shield treatment.

Implications:

  • Collagen shields offer a promising therapeutic option for managing rare shield ulcers in vernal keratoconjunctivitis.
