Summary

Repeat partial nephrectomy is feasible for hereditary renal cancer recurrence. While it causes a slight decrease in kidney function, most patients avoid dialysis and achieve acceptable oncological control.

Background:

  • Management options for recurrent renal tumors include repeat partial nephrectomy, ablation, or completion nephrectomy.

Purpose of the Study:

  • To evaluate the feasibility and outcomes of repeat partial nephrectomy on the same kidney unit.
  • To assess oncological control and renal function after repeat partial nephrectomy.

Main Methods:

  • Retrospective review of 51 repeat partial nephrectomy procedures in 47 patients (1992-2006).
  • Collected demographic, intraoperative, perioperative, and renal functional outcome data.
  • Compared pre- and post-operative renal function using a 2-tailed t-test.

Main Results:

  • 19.6% of cases had major complications or reoperations, with 1.9% perioperative mortality.
  • Successful repeat partial nephrectomy led to increased serum creatinine and decreased creatinine clearance and renogram split function (p<0.05).
  • 3.9% required long-term hemodialysis; 19.6% needed further surgery for recurrence or new tumors. 98% of patients survived at median follow-up.

Conclusions:

  • Repeat partial nephrectomy is technically feasible for recurrent renal tumors.
  • Despite functional decline, most patients maintain adequate kidney function, avoiding dialysis.
  • This procedure offers acceptable oncological outcomes for recurrent or de novo renal tumors.
