Background:

Appendiceal adenocarcinoma is a rare malignancy.
Bladder invasion by appendiceal tumors is an uncommon presentation.
Distinguishing secondary bladder tumors from primary ones can be challenging.

Observation:

Two cases of appendiceal adenocarcinoma invading the bladder are presented.
Both tumors initially presented as primary bladder carcinomas.
Cystoscopic biopsies revealed adenocarcinomatous tissue, with laparotomy confirming secondary bladder involvement.

Findings:

One patient underwent extensive surgical resection (appendix, cecal wall, bladder wall) and postoperative irradiation, remaining disease-free for 10 years.
The second patient was treated with right colectomy and segmental bladder resection but developed diffuse peritoneal recurrence and died 6 years later.

Implications:

This highlights the importance of considering extragenital origins for bladder tumors.
Aggressive surgical management and adjuvant therapies may improve outcomes for appendiceal adenocarcinoma with bladder invasion.
Further research into optimal treatment strategies for this rare condition is warranted.
