Summary

Late-onset herpes simplex keratitis can occur after corneal transplant (keratoplasty), even without prior infection history. Prompt topical antiviral treatment is effective for these graft infections.

Background:

  • Herpes simplex keratitis (HSK) is a common cause of corneal blindness.
  • Keratoplasty is a surgical procedure to replace damaged cornea.

Observation:

  • Three patients developed HSK 3-11 months post-keratoplasty.
  • Patients had no prior history of herpetic infections.
  • Dendritic keratitis occurred while on topical corticosteroids.

  • HSK should be considered in late-onset epithelial defects after keratoplasty.
