  • A 49-year-old man presented with a 20-year history of an asymptomatic reticular eruption.
  • The rash featured orange-red patches with reticular margins and central atrophy on the trunk and upper limbs.
  • Skin biopsies revealed elastophagocytosis and focal loss of elastic fibers.

Findings:

  • Histological examination confirmed elastophagocytosis and inflammation.
  • Elastin stains showed a loss of elastic fibers in the reticular dermis.
  • The case was consistent with mid-dermal elastolysis.

Implications:

  • This case highlights a rare reticular presentation of mid-dermal elastolysis.
  • It broadens the understanding of the clinical spectrum of dermal elastolytic disorders.
  • Further research may elucidate the etiology and pathogenesis of this condition.
