Summary

This study shows a modified two-port multiport illumination system (MIS) is safe and effective for posterior segment surgeries, with good visual outcomes and few complications. The MIS offers a comfortable option for vitreoretinal procedures.

Background:

Standard vitrectomy often involves multiple ports, potentially increasing surgical complexity and patient discomfort.
Minimally invasive surgical techniques are continually being developed to improve patient outcomes and reduce complications in ophthalmology.

Purpose of the Study:

To assess the safety and efficacy of a novel two-port multiport illumination system (MIS) in various vitreoretinal surgeries.
To determine the feasibility of using a modified MIS for pars plana vitrectomy.

A mean follow-up period of 3 months.

Main Results:

Visual acuity stabilized in 65.5% of eyes, improved in 24.5%, and deteriorated in 9.8%.
No intraoperative complications were reported; postoperative complications included hypotony (12 eyes), rehemorrhage (2 eyes), and choroidal detachment (1 eye).
Five cases necessitated conversion to a standard three-port vitrectomy; mean intraocular pressure decreased postoperatively.

Conclusions:

The modified MIS, utilizing a two-port sclerotomy, presents a safe and effective option for posterior segment surgeries.
This system offers a comfortable alternative for vitreoretinal procedures, despite some identified limitations.
Further research may explore broader applications and long-term outcomes of this minimally invasive approach.
