Summary

Flow cytometric immunophenotyping can distinguish marginal zone lymphoma (MZL) subtypes. CD11c expression identifies splenic MZL (SMZL), and CD19 levels help differentiate MZL from follicular lymphoma (FL).

Area of Science:

  • Hematology
  • Oncology
  • Immunology

Background:

  • Marginal zone B-cell lymphoma (MZL) encompasses splenic (SMZL), nodal (NMZL), and extranodal MALT types.
  • Flow cytometric immunophenotyping (FCI) is crucial for diagnosing MZL, especially when morphology is insufficient.
  • Differentiating MZL from CD10-negative follicular lymphoma (FL) is a key diagnostic challenge.

Purpose of the Study:

  • To identify immunophenotypic markers specific to MZL and its subtypes using FCI.
  • To establish methods for distinguishing MZL from CD10-negative follicular lymphoma (FL).

Main Methods:

  • Retrospective review of FCI studies for 31 MZL cases and 31 FL cases.
  • Analysis of antigen expression patterns and intensity (mean channel fluorescence - MCF).

  • Statistical comparison of antigen expression between MZL and FL groups.
  • Main Results:

    • All MZL and FL cases expressed CD19, CD20, and CD45.
    • CD11c expression was significantly higher in SMZL (57%) compared to other MZL subtypes (8%).
    • Brighter CD19 expression (mean MCF 455.3) was observed in MZL compared to FL (mean MCF 166.9).

    Conclusions:

    • MZL exhibits typical pan-B-cell antigen expression.
    • CD11c expression is a strong indicator for splenic MZL (SMZL).
    • Combined analysis of CD19, CD11c, and CD38 expression effectively differentiates MZL from CD10-negative FL.
