Liquid-based cytology for cervical cancer screening in Denmark significantly increased the detection of precancerous lesions and reduced unsatisfactory samples. This method offers advantages over conventional Papanicolaou smears, improving cervical cancer prevention.

Background:

  • Denmark has a long-standing cervical cancer screening program.
  • Despite organized screening, approximately 150 Danish women annually succumb to cervical cancer.
  • Current cervical sample preparation methods include conventional Papanicolaou smear and liquid-based cytology.

Purpose of the Study:

  • To compare the effectiveness of liquid-based cytology (LBC) with conventional Papanicolaou smear for cervical cancer screening.
  • To evaluate changes in diagnostic categories and sample adequacy after adopting LBC.

Main Methods:

  • Retrospective comparison of data from the Copenhagen Municipality cervical cancer screening program.
  • Transition from conventional Papanicolaou smear to SurePath liquid-based cytology in 2002 at Hvidovre Hospital.

Main Results:

  • A 64.3% increase in "atypical cells" and a 41.2% increase in "cells suspicious for malignancy" were observed with LBC.
  • The rate of "unsatisfactory for evaluation" tests decreased by 84%.
  • Follow-up histology indicated no significant increase in false positive results.

Conclusions:

  • Liquid-based cytology enhances the detection of cervical precancerous lesions.
  • LBC significantly reduces the proportion of unsatisfactory samples without increasing false positives.
  • The study suggests liquid-based cytology offers notable advantages over conventional smear methods for cervical cancer screening.
