Summary

Iridium-192 interstitial brachytherapy combined with wide local excision offers an 85% local control rate for soft tissue sarcomas. Intraoperative brachytherapy yielded superior results compared to postoperative implants.

Background:

Purpose of the Study:

  • To evaluate the efficacy of Ir-192 interstitial brachytherapy as an adjunct to WLE for soft tissue sarcomas.
  • To assess its role as a function-preserving treatment strategy.
  • To compare outcomes based on the timing of brachytherapy (intraoperative vs. postoperative).

Main Methods:

  • A cohort of 20 patients with soft tissue sarcomas underwent WLE and Ir-192 interstitial brachytherapy.
  • Brachytherapy was performed intraoperatively in 16 patients and postoperatively in 4.
  • Eleven patients also received external beam radiotherapy.

  • The overall local control rate was 85% (17/20) after a mean follow-up of 27 months.

Conclusions:

  • Wide local excision with low-dose-rate Ir-192 interstitial brachytherapy is an effective strategy for soft tissue sarcomas, achieving an 85% local control rate.
  • Intraoperative brachytherapy demonstrates superior local control compared to postoperative brachytherapy.
  • This combined approach shows promise for function-saving treatment in soft tissue sarcoma management.
