Summary

Bilateral testicular epidermoid cysts are rare but benign. Characteristic ultrasound findings enable testis-sparing surgery, avoiding orchidectomy.

Background:

Epidermoid cysts of the testis are rare, benign germ cell tumors, accounting for 1-2% of benign testicular masses.
While unilateral cases are common, bilateral occurrences are exceptionally rare, with only three prior reports in the literature.

Observation:

This report details a rare case of bilateral testicular epidermoid cysts.
The patient presented with characteristic ultrasonographic features suggestive of the condition.

Findings:

Characteristic ultrasound (US) findings were crucial in diagnosing bilateral epidermoid cysts.
These US features facilitated a testis-sparing enucleation, a conservative surgical approach.
