Summary

This study compared Starr-Edwards and Bjork-Shiley mitral valve prostheses in pediatric patients. Both improved functional class, but Starr-Edwards had a higher thromboembolism rate.

Area of Science:

  • Cardiology
  • Cardiovascular Surgery
  • Biomedical Engineering

Background:

  • Mitral valve disease in pediatric patients requires prosthetic intervention.
  • Long-term outcomes of different mitral valve prostheses in children are crucial for clinical decision-making.

Purpose of the Study:

  • To compare the long-term efficacy and safety of Starr-Edwards (6120) and Bjork-Shiley mitral valve prostheses in patients under 16 years of age.
  • To evaluate clinical improvement, hemodynamic changes, and complication rates associated with each prosthesis type.

Main Methods:

  • Long-term follow-up of two patient groups (N=74 Starr-Edwards, N=36 Bjork-Shiley) under 16 years old.
  • Assessment of clinical status using New York Heart Association criteria.
  • Cardiac catheterization to evaluate pulmonary pressures (PASP, PAWP) and complication incidence (thromboembolism, endocarditis, dysfunction, death).

Related Experiment Videos

Main Results:

  • Both prostheses significantly improved functional class (p < 0.006).
  • Significant reductions in pulmonary arterial systolic pressure (PASP) and pulmonary arterial wedge pressure (PAWP) were observed in both groups post-implantation.
  • A statistically significant higher incidence of thromboembolism was noted in the Starr-Edwards group (p = 0.037).

Conclusions:

  • Both Starr-Edwards and Bjork-Shiley mitral valve prostheses offer significant clinical and hemodynamic benefits in pediatric patients.
  • The Starr-Edwards prosthesis is associated with a higher risk of thromboembolism compared to the Bjork-Shiley prosthesis in this pediatric cohort.
