  • Emergency angiography revealed multivessel coronary artery narrowing during chest pain, relieved by nitroglycerin.
  • Diagnosis of variant angina (coronary vasospasm) was confirmed.
  • Beta-blocker therapy was discontinued and replaced with calcium channel blockers, resolving symptoms.

Implications:

  • Coronary vasospasm should be considered in patients with recurrent angina post-RNO.
  • Beta-blockers can be detrimental in variant angina and should be used cautiously.
  • Prompt diagnosis and appropriate medical management are crucial for favorable outcomes.
