Calcium hydroxide effectively induced apical root development and periapical repair in a young patient with pulp necrosis and an incompletely formed root. This apexification treatment resolved the abscess and promoted significant root lengthening over 8 months.

Background:

  • Dental trauma in children can lead to pulp necrosis and arrested root development.
  • Incomplete root formation presents challenges in endodontic treatment, particularly with associated periapical lesions.

Observation:

  • A 10-year-old male presented with an acute dentoalveolar abscess and a non-vital, immature tooth (tooth 11) with apical radiolucency.
  • Initial treatment involved urgent management of the abscess followed by chemo-mechanical debridement.

Findings:

  • Calcium hydroxide paste was used for apexification over 8 months, with regular changes.
  • Radiographic evaluation showed complete foramen closure, resolution of periapical radiolucency, and 5 mm of additional root development.
  • Long-term follow-up (3 years) confirmed normal periapical tissues and absence of symptoms.
