Context:

Laparoscopic appendectomy is not widely accepted for complicated appendicitis due to concerns about intra-abdominal abscesses.
Complicated appendicitis includes gangrenous or perforated cases.

Purpose:

To compare the morbidity of open versus laparoscopic appendectomy in patients with complicated appendicitis.
Evaluate outcomes including operation time, hospital stay, wound infection, and intra-abdominal abscesses.

Summary:

A prospective study of 107 patients with complicated appendicitis found lower overall morbidity and wound infection rates with laparoscopic appendectomy compared to open surgery.
Specifically, gangrenous appendicitis showed significantly lower morbidity with laparoscopy (p=0.014).
Perforated appendicitis also demonstrated reduced overall morbidity (p=0.046) and wound infection (p=0.004) with the laparoscopic approach.

Impact:

Laparoscopic appendectomy is a safe procedure for complicated appendicitis.
This technique offers potential clinical advantages over conventional open surgery.
Findings may encourage wider adoption of laparoscopic approaches for complex appendicitis cases.
