Background:

  • Whipple's disease (WD) frequently involves the gastrointestinal tract, yet its endoscopic presentation is poorly documented.
  • This study aimed to detail the endoscopic and pathological features of WD.

Observation:

  • Endoscopic findings in WD include mucosal edema, discoloration, erythema, subepithelial hemorrhages, and megaduodenum.
  • Magnification endoscopy revealed flattened villi, villi filled with white material, and internal white ring-like structures.

Findings:

  • Histologically, WD is characterized by foamy macrophages with coarse granular cytoplasm, strongly positive for periodic acid-Schiff (PAS).
  • A wide spectrum of endoscopic appearances was observed in patients with WD.

Implications:

  • Esophagogastroduodenoscopy with duodenal biopsies is an accurate diagnostic method for gastrointestinal WD.
  • Magnification endoscopy improves visualization of mucosal details, aiding in WD diagnosis.
