Background:

  • Peritoneal dialysis is a treatment for chronic renal failure.
  • Diabetic nephropathy and NSAID nephropathy are common causes of chronic renal failure.
  • Calciphylaxis (calcific uremic arteriolopathy) is a rare but serious complication in patients with chronic renal failure.

Observation:

  • A 51-year-old woman undergoing peritoneal dialysis presented with firm subcutaneous plaques.
  • A Technetium-99m MDP bone scan was performed to evaluate the plaques.
  • The scan revealed extensive superficial tracer localization and visceral tracer activity.

Findings:

  • The scintigraphic findings were characteristic of calciphylaxis.
  • This condition involves metastatic calcification, primarily in subcutaneous tissues.
  • It is associated with painful ulcerations, high morbidity, and mortality.

Implications:

  • Early diagnosis and treatment of calciphylaxis are crucial.
  • Sodium thiosulfate demonstrated significant improvement in scintigraphic findings.
  • This highlights the potential of sodium thiosulfate in managing calciphylaxis.
