Summary

This case report details the first known instance of sinonasal paraganglioma in an HIV-positive patient, emphasizing prompt diagnosis for this rare tumor. Early detection and surgical removal led to a successful outcome with no recurrence.

Background:

  • Sinonasal masses can present with diverse etiologies, requiring thorough diagnostic evaluation.
  • Paragangliomas are rare neuroendocrine tumors that can occur in the sinonasal region.

Observation:

  • A 39-year-old HIV-positive female presented with a bleeding, lobulated left nasal mass requiring multiple blood transfusions.
  • Diagnostic workup confirmed HIV infection and sinonasal paraganglioma, with a CD4+ count of 487 cells/mm3.

Findings:

  • Histopathological examination confirmed the diagnosis of paraganglioma.
  • Urinary vanillylmandelic acid assay and echocardiography were within normal limits.
  • Complete tumor excision was achieved via medial maxillectomy.

Implications:

  • This is the first reported case of sinonasal paraganglioma in an HIV-positive individual globally and the first in Africa.
  • The findings underscore the importance of a high index of suspicion for sinonasal paraganglioma in patients with bleeding nasal masses, irrespective of HIV status.
  • Further research is needed to elucidate the potential relationship between HIV infection and the development of sinonasal paraganglioma.
