Background:

  • Brunner's gland hyperplasia (BGH) is a common benign duodenal lesion.
  • It is incidentally discovered in approximately 2% of upper gastrointestinal endoscopies.

Observation:

  • A 34-year-old asymptomatic female presented with a large, 12-cm duodenal lesion.
  • The lesion had a distinctive 'elephant's proboscis' morphology.

Findings:

  • Surgical excision confirmed the lesion as Brunner's gland hyperplasia.
  • This represents a rare, large presentation of an otherwise typically incidental finding.

Implications:

  • Surgical intervention can be curative for symptomatic or large BGH.
  • Endoscopic and surgical management strategies for BGH require careful consideration based on size and presentation.
