Summary

This report details the first documented case of granulocyte-colony stimulating factor (G-CSF)-producing rectal cancer. This rare G-CSF-producing cancer has a very poor prognosis, highlighting the need for further research.

Background:

  • Granulocyte-colony stimulating factor (G-CSF)-producing cancers are typically found in the lung.
  • G-CSF-producing colorectal cancer (CRC) has not been previously reported in English literature.

Observation:

  • A 57-year-old male presented with rectal cancer and concurrent liver tumors.
  • Post-surgery, the patient developed a significantly elevated white blood cell count (81,000 cells/microL).
  • Elevated serum G-CSF levels (840 pg/mL) and positive immunohistochemistry confirmed G-CSF production by the tumor.

Findings:

  • The case was diagnosed as an undifferentiated G-CSF-producing rectal carcinoma with liver metastases and peritoneal dissemination.
  • Standard chemotherapy (Modified-FOLFOX6) was ineffective against the metastatic disease.
  • The patient experienced rapid disease progression and succumbed to complications within 2 months.

Implications:

  • This case represents the first documented instance of G-CSF-producing rectal cancer.
  • The extremely poor prognosis observed underscores the aggressive nature of this rare malignancy.
  • Further investigation into G-CSF-producing rectal cancer is warranted to understand its pathogenesis and explore potential therapeutic strategies.
