Background:

  • Blood pressure (BP) fluctuations are common during prone lumbar spinal surgery.
  • Noninvasive cardiac output monitoring can help elucidate causes of BP decrease.

Purpose of the Study:

  • To investigate the causes of decreased BP during prone lumbar spinal surgery.
  • To utilize a noninvasive cardiac output monitor for hemodynamic assessment.

Main Methods:

  • Hemodynamic status of 20 patients undergoing elective prone lumbar spinal surgery was monitored using Impedance Cardiography.
  • Data including heart rate (HR), mean BP, cardiac index (CI), stroke volume (SV), and systemic vascular resistance (SVR) were collected at various time points.
  • Statistical analysis included Friedman's test and paired t test.

Main Results:

  • Significant differences in mean BP, SV, CI, and HR were observed (p < 0.05).
  • CI and SV decreased significantly after patients assumed the prone position.
  • Mean BP also decreased significantly, with further reduction after 1 hour of surgery.

Conclusions:

  • Reduced stroke volume (SV) and cardiac index (CI) are the primary contributors to blood pressure decrease in the prone position during lumbar spinal surgery.
