Summary

A new cough test effectively screens for silent aspiration (SA) in dysphagia patients. Combining it with the modified water swallowing test (MWST) creates a reliable bedside system for identifying aspiration risks.

Background:

Purpose of the Study:

  • To evaluate the efficacy of a cough test for screening SA.
  • To develop and validate an accurate screening system for SA by combining the cough test with the modified water swallowing test (MWST).

Main Methods:

  • A cohort of 204 patients with suspected dysphagia underwent a cough test and instrumental assessment (videofluorography or videoendoscopy).
  • A subset of 107 patients also completed the MWST.
  • Sensitivity and specificity of the cough test for SA detection were calculated.

  • The cough test demonstrated high sensitivity (0.87) and specificity (0.89) for detecting SA.
  • The combined screening system (cough test + MWST) showed good agreement with instrumental assessments in classifying patients' swallowing safety.
