Summary

Partial anomalous pulmonary venous return (PAPVR), a rare congenital heart defect, can present unusually. This case highlights a solitary pulmonary nodule as the initial symptom of right upper lobe PAPVR without an atrial septal defect.

  • Partial anomalous pulmonary venous return (PAPVR) is a rare congenital heart anomaly affecting 0.4-0.7% of the population.
  • It commonly involves abnormal pulmonary venous drainage into the systemic venous circulation, often associated with an atrial septal defect (ASD).

Observation:

  • A young soldier presented with exertional dyspnea unresponsive to treatment and an 8.5-mm solitary pulmonary nodule.
  • Diagnostic imaging confirmed PAPVR.

Findings:

  • This case represents the first documented instance of a solitary pulmonary nodule as the initial clinical manifestation of right upper lobe PAPVR draining into the superior vena cava.
