Background:

  • Obstructive sleep apnea (OSA) is a prevalent condition associated with significant health risks.
  • Transient heart rate changes during obstructive apneas have been explored as a potential screening method for sleep disordered breathing (SDB).
  • Current screening methods often require specialized equipment and dedicated sleep studies.

Purpose of the Study:

  • To prospectively evaluate an automated electrocardiogram (ECG)-based screening tool for sleep disordered breathing (SDB).
  • To compare the diagnostic performance of the automated ECG tool against simultaneous polysomnography (PSG).

Main Methods:

  • An automated algorithm was applied to single-channel ECG data from 92 subjects undergoing overnight polysomnography.
  • The algorithm estimated the apnea-hypopnea index (AHI).
  • Likelihood ratios were calculated using established AHI thresholds (<5 for absence, >=15 for presence of SDB).

  • The automated ECG algorithm demonstrated a positive likelihood ratio of 2.16 and a negative likelihood ratio of 0.08.
